Concerned about Falling?
Our team can help you evaluate modifiable risks for falling and provide suggestions on ways to reduce your risk. We offer FREE fall risk screening based on the established STEADI fall risk screening process recommended by the Centers for Disease Control and Prevention. The screening is coordinated in collaboration with the statewide Iowa Fall Prevention Coalition and the Iowa Community HUB. What is Involved in Screening?
The screening involves completing a brief survey and completion of a few functional fitness tests. The standardized Stay Independent Survey provides information on your history and perceptions, but the functional tests below help to evaluate your balance, strength and gait.
What Will I Learn?
The screening will provide you with a personalized report and will help refer you to appropriate programs and resources based on your risks (See Sample Below) (download sample here). How Do I Reduce Risks?
